Our league requires that all players have a current color photo for pass cards which are issued by the league.  The league also requires that each club has proof of age on file for all players.

Please upload your player’s photo (color head and shoulders picture)  or proof of age (birth certificate or passport) using the following form. Please no hats or action shots.
